ALAMEDA COUNTY HEALTH CARE
Our Results Framework
We seek measurable impacts on five core results.
No single organization can fix the complex social problems in our schools, communities, and society. We believe coalitions and partnerships are essential for achieving measurable and lasting community change.
Our results framework defines five core goals, for the Center and our partners, that support our objective for all our youth to graduate from high school healthy and ready for college and career. To measure our collective impact on these results, we have selected lead indicators to track our combined progress over time. Annual results for indicators will be available on our forthcoming Results Scorecard.
Our individual program evaluations measure the impact of specific programs and services for specific populations served. Learn more…
Result 1: Children Are Physically, Socially, and Emotionally Healthy.
Good health during childhood supports students' ability to thrive in and outside the classroom, prepare for college and career, and lead healthy lifestyles in adulthood. We seek to support the healthy development of the whole child, including their physical, social, and emotional health.
Result 2: Children Succeed Academically.
Educational attainment is closely correlated with health outcomes in adulthood. Young people with higher levels of educational attainment are more likely to have access to better job opportunities and lead longer and healthier lives.
Results 3: Environments Are Safe, Supportive, and Stable.
The environments children live in shape the opportunities they have to be healthy and academically successful. Safe, supportive, and stable homes, schools, and communities are critical for supporting all young people to grow and thrive.
Results 4: Families Are Supported and Supportive.
Families are fundamental to children’s academic and healthy development. Children thrive when families have access to basic supports, such as quality jobs, affordable housing, and health care; and when families have meaningful opportunities to engage in their child’s development and education.
Results 5: Systems Are Integrated and Care is Coordinated and Equitable.
Deep collaboration is essential for eliminating inequities in our communities. Our work is guided by the core belief that it will take a coordinated, cross-sector coalition of partners working together to create more equitable conditions for all children to thrive.
