ALAMEDA COUNTY HEALTH CARE
Communities of Practice
We convene and facilitate communities of practice across various areas of work, such as school health centers and school-based behavioral health. These communities of practice foster collaborative learning and create forums for sharing best practices and resources that help to support, sustain, and improve the work.
For example, our staff convenes monthly meetings with our network of school health centers to share challenges and successes, problem-solve, and make improvements based on examination of evaluation data. We also facilitate communities of practice for the county’s school-based behavioral health providers to share best practices from peers and emerging research, and to support the development of a more comprehensive and integrated system of care.
